It is very possible that you've experienced a scenario in which you either hadn't finished drinking the perk, where overwhelmed + multi-tapped, had already damaged yourself with an explosive, or not fully healed from previous hits. Considering the map's constrained playspace, it's very easy to be overwhelmed.
