Since the automation you created to generate the diagrams follows Prereqs it has both of these inconsistencies but it is 100% reliable in terms of what the game is actually asking for as opposed to what it says its asking for.

I imagine that doing these in the perfect order the first time wasn't Ryukishi07's intention. He probably meant for people to work it out for themselves—or among themselves—by some sort of analysis and maybe a little guesswork. Maybe he gave hints, too (though that's just a guess).
The only situation where you're not allowed to make mistakes is when trying to get the secret; in this case, you have to finish the VN once, then view the Fragments in a very specific order (not shown in this diagram), which is their chronological order. You don't necessarily have to view the Fragments twice. Instead, you can do them in the correct order the first time, make a save, proceed to the third act and finish, then reload the save; the secret will then be available. However, this does mean knowing the correct order early on, which you might not want.
