IPECAC mod is one of the original large content expansions to the game and started as far back as 2016. This mod is still going strong and supports the latest versions of the game. This is also one of the most expansive mods, basically a free DLC and adds a new character, almost a hundred new items, a few thousand rooms, and dozens of new enemies and trinkets. This mod adds some very fun items and mechanics, and overall it's a high quality, well balanced and awesome mod that I definitely recommend.

This is a different kind of expansion, as instead of expanding the base game experience, it adds an entire new gamemode into the game with enough brand new mechanics and gameplay that it might as well be an entire new separate game! This is one of the most unique modded characters I've seen, with a totally unique and fun attack mechanic. Instead of items acting as upgrades to your tears like any other character, you attacks are completely dependant on the flask you hold which are randomly generated and commonly dropped. Playing this character basically turns the game into a looter shooter! There's even a rarity system and you can swap between 2 equipped flasks.
