Widescreen, FOV, FPS and Brightness fix
To fix the main issues with this game right click Call of Duty (2003) and select Properties. Underneath the Launch Options copy and paste the following:

+set r_mode -1 +set r_customwidth 3840 +set r_customheight 2160 +set r_customaspect 1.7 +set cg_fov 105 +seta com_maxfps 120 +seta r_overBrightBits 0.3 +seta com_introplayed "1" +bind F12 "" +seta com_hunkmegs "512"

Ensure you:
Adjust: '+set r_customwidth' and '+set r_customheight' to match your screens resolution (3840x2160 by default)

Adjust: '+set cg_fov' to whatever FOV you want (105 by default)

Adjust: '+seta com_maxfps' to match your screens refresh rate (120hz by default)

A few extra commands are included to skip the intro logos, unbind F12 by default as the game's own screenshot feature gives an error on resolutions above 1440p and to fix the 'Hunk_AllocateTempoaryMemory' error when trying to quick save or load a level.

With these simple commands the game is playable from start to finish at a high resolution and refresh rate. These are automatically applied by Steam whenever you install the game so you won't have to mess around with ini editing every time and they also apply to United Offensive as long as you install Call of Duty (2003) first.

Zoomed in image fix:
If you have a 4K monitor and are using the Windows scaling feature above 100% the game may appear zoomed in like this:
To fix this go into settings, display and set the scaling to 100% while playing the game.
(Optional) Anisotropic Filtering, Anti Aliasing, Colour Deband and Upscaled Textures.
Anisotropic Filtering and Anti Aliasing:
To apply 16x Anisotropic Filtering and 8xMSAA open the Nvidia Control Panel go to Manage 3D Settings, Program Settings, Add and select Browse. Locate where you installed Call of Duty and select 'CoDSP.exe' in the main folder. Put Anisotropic Filtering at 16x, set the Anti Aliasing mode to 'override any application settings' and then to 8x. If the Anti Aliasing options are greyed out you'll have to force it with Nvidia Profile Inspector[github.com].

Reshade SMAA:
Alternatively, you can download Reshade[reshade.me] for SMAA and combine the two. After Reshade is downloaded, click on the setup and like previously locate where you installed Call of Duty and select 'CoDSP.exe' in the main folder. After you selected the exe, click Next, OpenGL, tick 'sweetFX by CeeJay', click Next again and it should be done. Launch the game and press the 'Home' button to open up the Reshade menu, skip tutorial and use the the search bar in the Home section to look for SMAA and tick it. Disable SMAA if textures appear blurry.

Reshade Colour Deband:
Reshade's sweetFX collection that was used for SMAA also has a colour deband option to get rid of colour banding mainly visible on the skyboxes and smooth them out. As before use the search bar, look for 'Deband' and tick it.

Upscaled Textures:
If you want higher res textures that are faithful to the original game download the HD Texture Pack by CountBuggula on Moddb or Nexus[www.nexusmods.com]. To install this mod extract the files into \Call of Duty\main\ directory.
