Preface
This guide will show a list of every death in the game needed to complete this achievement. These are all listed in order. I recommend setting up a timer for some of these deaths as 5 of them require a LOT of waiting and doing nothing.
Caldoria Start
Uncreated: Instead of going to "TSA" from the Global Transporter, pick either "Hard Rock" or "Flagstaff".
(I tried waiting around to see if it was triggered by not going to work, but after an hour of waiting in Caldoria, I didn't die. I did get a first late warning after 5 minutes, and a 2nd warning after 10 minutes, but nothing after that.)
TSA Regular
Uncreated: Once the temporal rip starts, don't do anything for 10 minutes. You can watch the screen and see the wave slowly move to the right. Once it reaches the 2318 dot, the death should trigger.
Prehistoric
Dinner Time: Find the cave with the dinosaur eggs and move towards them.

Face Plant: Fall off a cliff.

Stranded: Run out of energy. (death after TWO HOURS OF IDLING)
TSA Alternative
Blown Away: Do not move the robots and just exit the door.
Mars
Do NOT get the oxygen mask until you get Suffocated #1.

Incarcerated: In the room with the minecart key, open one of the doors with the two guards playing rock-paper-scissors.

Wrong Turn: During the minecart sequence, take a turn that ends up killing you.

Eliminated: Don't move out of the way of the robot when asked to.

Road Kill: After you move out of the way of the robot before the reactor, go back to the minecart room and step on the tracks.

Toasted: Run out of energy inside the Mars reactor. (not a long wait this time, only 20 minutes of waiting. Make sure you die INSIDE the reactor.)

Vaporized: Attempt removal of the card bomb, or fail disarming the Mars card bomb.

You're History: Enter the Mars maze without disabling the card bomb.

Suffocated #1: Enter the Mars maze WITHOUT the oxygen mask after getting the Card Bomb, and wait shortly for the death.
YOU CANNOT PICK UP THE OXYGEN MASK AT ALL. IF YOU HAVE IT, EVEN IF IT'S EMPTY, IT WILL NOT TRIGGER THIS DEATH.

Suffocated #2: Enter the Mars maze after getting the Card Bomb and spend enough time inside for your OXYGEN to run out. Make sure you have the oxygen mask or you will get #1 instead.
(I started timing this from when I entered the maze and it took 30 minutes. I got 50%, 25%, and 5% warnings.)

Pulverized: Die by one of the Mars maze robots.

Shafted: Fall down the end of the Mars maze instead of climbing onto the basket. If you accidently get on the basket, just press up as you are riding up and you will fall off.

Pulverized: Stay on the basket and ride the basket up to your death. If you accidently get off, turn around and get back on.

Wrong Door: Instead of going to your ship at the end, enter a different door and fall out of it.

Crash & Burn #1: Crash during the initial chase sequence before going to space.

Crash & Burn #2: Die during the shooting sequence in space.
NORAD
Incarcerated #1: Remove the sleeping gas canister and move down the hallway. The man will wake up.

Incarcerated #2: Run out of energy. (TWO HOURS OF IDLING)

Gassed: Leave the sleeping gas in but do not have your oxygen mask on. Wait it out and you will fall asleep (takes about 1 1/2 minutes of waiting).

Sunk: Get hit by a torpedo in the submarine chase (I don't think the first torpedo kills you if you get hit).

Crushed: Die by the robot at the end while standing in front of the door you usually use to raise the pressure.

Eliminated: Die by the robot at the end while sitting at the claw arm controls.
WSC
Poisoned: Do not remove the dart from your body (or remove it and wait 20 minutes).

Expelled: Get caught by any employee.

Blown Away #1: Have somewhat low energy before opening the door with the crowbar, then have that robot kill you from a distance (do not wear the shield biochip).
(Let the poison from the dart take away most of your health until roughly 10% left, should take about 15 minutes. Drink the antidote, leave the room, rush to the crowbar door, SAVE in case you need less energy, open the door, then step forward and you should die.)

Blown Away #2: Get shot by the robot on the catwalk at the end.
Caldoria Ending
Mooed Down: Sit in the transporter, hold E and click Other... (only works after all rips have been solved)

In Your Face: Do not wear your Shield biochip when blasting open the roof door.

Assassinated: After blowing the door, turn left and walk forward.

Shot by Sinclair: Have Sinclair shoot you.

Nuked: After shooting Sinclair, leave the roof.

In Mars, I think you can just go into the Gear room to get the Suffocated #2 death (No Gas Mask in inventory). You also do this by going into the Maze and standing still (so as not to trigger the explosion).

However, I DID get the final death by allowing time to run out whilst waiting on the Mars gantry. Presumably this would work anywhere there was oxygen, so you don't get the "Suffocated" death instead

Also, running out of energy in NORAD *didn't* count as another death

I'm not seeing any difference between "Suffocated #1" and "Suffocated #2". I did both, and my count didn't go up on the achievement.

Turns out running out of energy in NORAD IS another death, which means another 2 hours of waiting. It shares the same death screen as getting caught by the first guard.

If you are missing one, like I was, Mars Suffocated #1's method of unlocking is now different. Try getting that one again.

This guide is now complete.

If you open the door to the catwalk in the WSC and wait, the robot will shoot Castillo, which I think counts as another death.

I've been having the same problems with Gassed #2 also, as it's counting as Incarcerated even with the canister in.

I put the guide back into incomplete mode, there is still an ending missing. I spent 4 hours today trying to confirm that last one and it was false.

well blitter mentioned you can get "stranded in NORAD", so I decided to wait 2 hours and running out of energy is just the same ending as waking up the guy at the start.
